1. Application Form.
Please COMPLETE THE APPLICATION FORM IN English OR Italian (HSINyin is ACCEPTED). If THE applicant is a Chinese citizen, please SIGN the FORM IN Chinese. Please attach a recent two-inch color photo to the application form.
2. Passport.
Valid passport, valid for at least 90 days after the visa expires. The passport must have the Chinese signature of the passport holder and a copy of the basic information page (with photo page) of the passport.
If the applicant has the same type of expired passport or type of passport (whether expired), please show it when applying for the visa.
3. If the passport has a visa, please submit a copy of the visa page.
Certificate of Registration The certificate of registration of the University shall include: the type of course taken, the language in which the course is taught, whether the course includes Italian training, the date and duration of the course, and the tuition fee for the entire course.
If ACCOMMODATION AND insurance are PROVIDED, PLEASE INCLUDE THE ADDRESS AND TELEPHONE NUMBER OF YOUR RESIDENCE ON YOUR registration CERTIFICATE and a copy of YOUR insurance policy, which must cover the entire period OF YOUR stay in Italy.
Certificate of Education Highest Certificate of Education: Please present the original certificate of education when applying for the visa, and translate the copy into Italian to the Ministry of Foreign Affairs of China for certification and then submit it to the visa Office.
Certificate of equivalent Education: Please present the original certificate of equivalent education issued by the Italian Consulate and provide a copy.
Note: Certificate of equivalent education will be issued. The Consulate needs notarial certificate of highest education and College Entrance Examination. Please translate it into Italian and submit it to the Ministry of Foreign Affairs of China for certification.
Certificates If you have a training certificate in Italian or English or training certificate related to the course of study in Italy, please provide it.
Letter of Introduction If you are a student, please provide the original letter of introduction from your parents’ Chinese company or institution.
The letter of introduction should be made out on the company’s head paper, stamped with the official seal and signed by the person in charge.
The letter of introduction must include the company’s address, telephone number, fax, parent’s job title and salary.
If your parents are owners of private enterprises, please show the original business license and provide a copy when applying for the visa.
